Telehealth $75

Receive personalized, professional care from the comfort of your home.
Telehealth visits are available for both new and established patients and are ideal for routine follow-ups, medication management, lab review, or non-urgent concerns.
All telehealth visits are cash-only, with payment due at the time of service.
A superbill can be provided upon request for possible insurance reimbursement.

New Patient Visit $225

Your first visit at 521 Wellness is focused on getting to know you — your health history, current medications, and wellness goals.
This extended appointment allows time to review your medical background, create a personalized care plan, and discuss preventive strategies for long-term health.


Duration: 45–60 minutes

Established Patient Visit $150

For returning patients, these visits are designed for ongoing management of chronic conditions, medication refills, or review of new symptoms or test results.
Your care plan is updated as needed, and education or preventive recommendations are reviewed.


Duration: 30 minutes (telehealth or in-person available)

Sick Visit $150

When you’re not feeling well, you shouldn’t have to wait days for care.
Sick visits address acute, non-emergent concerns such as cough, sinus symptoms, mild infections, stomach upset, rashes, or medication reactions.
These visits can often be completed via telehealth or home visit, depending on your symptoms and preference.

Same- or next-day appointments may be available.

Duration: 15-30 minutes (telehealth, in-person, and home visit available)

Home Visit

New Patient: $275

Est. Patient: $225

Sick visit: $200

Personalized primary care brought right to your door.
Home visits are ideal for adults and older adults who prefer or require care in their own home setting.

Each visit includes a comprehensive assessment, vital signs, medication review, and care plan discussion — with plenty of time for questions.

Physicals

Sports: $45

Employment: $55

Quick, convenient physicals for school, sports participation, or workplace requirements.
Each includes a review of medical history, vitals, and physical exam to ensure safety and readiness for activity or employment.

Documentation is provided at the time of the visit.

Available as in-office

All sports and employment physicals are cash-only, with payment due at the time of service.

We accept payment at the time of service. Insurance will be billed, and any applicable copay or coinsurance will be collected during your visit.

Superbill & Claim Submission
A detailed superbill will be provided along with a claim submission form for patients or their families to submit to their health insurance provider for possible reimbursement.

Laboratory & Imaging Services

  • Laboratory tests ordered during your visit will be billed directly by the lab to the patient’s insurance.

  • Imaging services (such as X-rays, ultrasounds, or CT scans) will be billed directly to the patient’s insurance by the imaging department or facility performing the test.

Hello, I’m Diana Bucher, MSN, APRN-CNP, AGPCNP, founder and provider at 521 Wellness.
I’m a certified Adult-Gerontology Primary Care Nurse Practitioner with over a decade of combined nursing experience across skilled nursing, primary care, and community-based settings.

I created 521 Wellness to bring back what healthcare should be — personal, accessible, and focused on genuine relationships. My approach blends preventive care, patient education, and individualized treatment to support each person’s unique health goals.

My philosophy of care:
