Telehealth visits are available for patients 18 and older — ideal for follow-ups, medication management, lab review, or non-urgent concerns.
All telehealth visits are cash-only, with payment due at the time of service.

Telehealth Weight Management $25
This 30-minute telehealth consultation is designed to help you learn more about our medically supervised weight management program and determine if it is the right fit for your goals. During this visit, you will have the opportunity to ask questions, review your health history, discuss expectations, and explore personalized treatment options.
A $25 consultation fee is required at the time of booking. This fee will be waived or refunded if you choose to enroll in the weight management program.

What to Expect During Your $25 BHRT Consultation
Your initial consultation is designed to help us better understand your symptoms, health history, and wellness goals. During this visit, we will:
Review Your Symptom Questionnaire: Discuss your current symptoms, medical history, and how you have been feeling.
Explore Treatment Options: Learn about BHRT, including hormone pellet therapy, potential benefits, risks, and available treatment options.
Discuss Laboratory Testing: Review the bloodwork needed to evaluate your hormone levels and other important health markers.
Review Required Health Screenings: Women must have a mammogram completed within the past 12 months before beginning pellet therapy. Additional screenings may be required based on individual health history. Men may require PSA testing.
Discuss Your Next Steps: Determine what additional testing or evaluation may be needed before developing an individualized treatment plan.

Your $25 consultation fee will be waived when you enroll in our BHRT program!
Important Payment Information
BHRT is a cash-pay service. Insurance is not accepted for BHRT consultations, hormone therapy, or pellet insertion procedures.
Laboratory testing, hormone therapy, and pellet insertion procedures are billed separately from the initial consultation. All applicable costs will be discussed before proceeding with treatment.

At 521 Wellness, we provide a structured, clinically supervised weight loss program using GLP-1 medications such as Semaglutide and Tirzepatide. Payment at Time of Service
All prices listed are cash visit rates, unless services are submitted to insurance for billing.
Payment is due at the time of service. If your visit will be billed to insurance, your copay or coinsurance will be collected at that time.
Insurance Billing & Reimbursement
If your insurance plan is billed, 521 Wellness will submit the claim on your behalf.
A superbill will also be issued along with a claim submission form for patients or families who wish to submit directly to their insurance for potential reimbursement.
Laboratory & Imaging Services
Lab tests ordered during your visit will be billed directly by the laboratory to your insurance provider.
Imaging services (such as X-rays, ultrasounds, or CT scans) will be billed directly by the imaging facility to your insurance provider.

Please ensure you are registering for the correct CPR course required for your employer, school, licensing board, or certification needs. While I am happy to provide general guidance regarding course options, it is ultimately the student’s responsibility to confirm they are selecting the appropriate course for their specific requirements.
When purchasing a CPR course, the exact training and certification selected at registration will be provided. If the course chosen does not meet your employer, school, or licensing requirements, refunds will not be issued, as course selection is the responsibility of the student. My goal is always to provide high-quality CPR and emergency response training in a professional and supportive learning environment.

Hello, I’m Diana Bucher, MSN, APRN-CNP, AGPCNP, founder and provider at 521 Wellness.
I’m a certified Adult-Gerontology Primary Care Nurse Practitioner with over a decade of combined nursing experience across skilled nursing, primary care, and community-based settings.
I created 521 Wellness to bring back what healthcare should be — personal, accessible, and focused on genuine relationships. My approach blends preventive care, patient education, and individualized treatment to support each person’s unique health goals.
